**Service Agreement for Neuro Fitness Program**

This Service Agreement ("Agreement") is entered into by and between Rooted, Wyld, and WELLthy w/ Dr. Chelsie ("Provider") and the participant ("Participant") of the Neuro Fitness Program ("Program"). By enrolling in the Program, Participant acknowledges and agrees to the following terms and conditions:

### 1. Nature of the Program

The Program is designed to support and enhance the Participant's mental and physical well-being through personalized coaching, exclusive app access, and ongoing support. It is intended for educational and informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment.

### 2. No Medical Advice

Participant acknowledges that the Provider is not replacing additional medical professional advice, and the services provided are not medical services. The Program does not replace the need for services provided by medical professionals, and Participant is encouraged to consult a healthcare provider for any medical concerns. The Provider does not guarantee any specific health-related outcome from participation in the Program.

### 3. Participant's Commitment and Responsibility

Participant acknowledges that their results and achievement of goals are directly related to their own activity, commitment, and application of the principles of the Program. The Participant is responsible for the effort and dedication required to achieve their personal goals. The Provider does not guarantee any specific results or outcomes from participation in the Program.

### 4. Acknowledgment of Risks

Participant acknowledges that all activities and practices related to the Program are undertaken at their own risk. The Participant agrees to release the Provider from any claims or liabilities of any kind arising out of the Participant's participation in the Program.

### 5. Confidentiality

The Provider agrees to maintain the confidentiality of the Participant's information and records in accordance with applicable laws and professional standards. Participant also agrees to respect the confidentiality of any information shared within the Program by other participants or the Provider.

### 6. Agreement to Terms

By enrolling in the Program, Participant agrees to the terms outlined in this Agreement and acknowledges that adherence to these terms is essential for the effectiveness of the Program and the achievement of its goals.

This Agreement represents the entire understanding between the Participant and the Provider regarding the Participant's enrollment in the Program and supersedes all prior agreements and understandings, whether written or oral.

**By proceeding with the enrollment, the Participant acknowledges that they have read, understood, and agreed to the terms and conditions of this Service Agreement.**
